I acknowledge that I do not have to consent in order to receive assistance or services from Georgia Access and that my refusal to consent will in no way affect the availability of the amount of assistance or services I receive from Georgia Access. I also acknowledge that this consent may be removed at my request but that until such consent is revoked, I may receive calls or text messages from Georgia Access at my wireless number.
